Genitals usually can be glanced upon at birth to determine sex, with rare exception. There is a very strong correlation between between the observed genitalia and the many other sex characteristics. That said, there are exceptions, so genitalia does not determine sex in all cases and I don’t see any good reason to indicate sex on a birth certificate anyway.
